Confidential Electronic Health Record (EHR) Information

Login ID and Password and password are required to access your EHR. Login ID and Password information are given to each patient at their yearly eye exam appointment as of January 1, 2018. If you do not have your login information you can contact our office by phone or online to request this, which will be mailed to you at the address of record.

  • You may click on each filename to view or download the file from your visit.
  • Be sure to logout of the Patient Portal when you are done.

Important notes:

  • The large green box in the Patient Portal allows you to send us a secure message. This is meant for confidential secure messages only. If you have a general question please call our office or use the “Ask us a Question” icon on our website and you will get a much faster reply.
  • Allow up to four days from your last visit for your complete record to be accessible.
